The opt out form can be picked up from the student benefits office which is located on the first floor of the University Center.
What are the fees?

There are two specific fees that all undergraduate students pay to the CSA for the Health Plan and the Dental Plan. The cost of the universal health plan is $125.00 and is not an option for students to opt-out of. The cost of the dental plan is $118.00 and is optional. Details are listed below on how to opt-out of our student dental plan.

Next please click on the left-hand side and select the Benefits Brochure and Benefits Booklet to download. All the benefits are listed within these documents and give clear descriptions of what the plans cover.

Also contained in these documents is information on where to ask questions on campus and where to send claim forms. How do I opt-out of the dental plan?

If you are a registered student in the Fall Semester and did not opt out last year, you have until September 25th, 2009 at 4:00pm to submit your completed opt out form with confirmation of alternate coverage (note: this includes co-op students on a Fall work term). If you are a registered student and completed your opt out form last year you are automatically opted out and are not required to opt out again this year. If you opted out last year and now require dental coverage (e.g. your alternative coverage is not provided this year), you must submit your completed opt in form to the on-site Student Benefits Plan Office. Refund cheques for Fall opt outs will become available at the beginning of November 2008 from the on-site Student Benefits Plan Office.

Why can I not opt-out of the health plan?

The student health plan provided by your CSA is a mandatory fee in an effort to keep supplemental health care affordable for all undergraduate students. Addtionally, at Student Health Services on campus is able to bulk purchase several drugs, saving students a huge amount of money in the future.

In an effort to use both this plan and your alternative coverage you can coordinate your claims to 'top up' alternative coverage. Please read through the detailed information on the aforementioned website to see how you can best integrate your coverage.

This plan is administered through our on-site Student Benefits Plan Office, located on the first floor of the University Centre. All claim forms, family application forms, benefit cards, and information brochures are available there. Your plan covers 100% of most medications requiring a prescription. Dispensing fees charged by the pharmacy are not covered by the Plan. The Plan is based on the National Formulary, which covers generic equivalent drugs where available.

Brief summary of health & dental plan coverage

Dental Plan:

  • 100% of one examination per year, including most x-rays, based on the current fee schedule
  • 100% of one cleaning and polishing, at one unit only and includes up to 2 units of scaling
  • 100% of one fluoride treatment per policy period
  • 75% coverage of most fillings
  • 75% coverage of extractions, limited to 2 wisdom teeth in any calendar year
  • $500 annual maximum per policy period, per person

Health Plan:
  • Most insulin, needles, syringes and chemical diagnostic aids for the treatment of diabetes.
  • Most antineoplastic drugs.
  • Most oral contraceptives.
  • A variety of paramedical services with a combined maximum.
  • In the event of illness or injury (after 15 day confinement), the cost of private tutorial services up to $15/hour for a maximum of $1,000 per disability.
